Illsutrate the pharmacokinetic and pharmacodynamic process


Problem: Patient factors that might have interfered with the pharmacokinetic and pharmacodynamic processes of a taking diphenhydramine (Benadryl) at home to help with sleep and is now asking for Benadryl to help the patient sleep.  However, the hospitalist decided to prescribe Zolpidem (Ambien) 10 mg at bedtime which was administered on the first night without any issues. After takin the second dose, the patient becomes increasingly agitated, confused, and combative. The night nurse tried to figure out whether the behavior was from Zolpidem or Oxycodone that the patient was taking for pain. Upon assuming care of the patient in the morning, I notified the physician that the patient was very confused and agitated which was unlike her behavior from the previous day. The physician ordered Lorazepam 0.5 mg (Ativan) intravenously to help with agitation which only made the situation worse as Mrs. MJ become more agitated and combative.
