If I recall correctly, the direct catalyst for the challenger accident was the O-Ring design coupled with the freezing temperatures prior to launch, which was and is an anomaly for Cape Canaveral. Nonetheless, the ensuing investigation opened up many different areas of concern and put NASA under a microscope. What was learned from this accident relevant to effective or ineffective leadership, and what did NASA do to reinvent itself after Challenger, if anything? Did NASA's culture change as a result of this first of 2 Space Shuttle disasters, and was the change sustained?
