Problem

De Peri, a 55-year-old aboriginal man, is hospitalised to the respiratory ward with sudden onset shortness of breath. Shortness of breath is associated with activity on exertion with productive cough. He had previously experienced symptoms that necessitated hospitalisation nearly six months ago due to an abrupt aggravation of chronic obstructive pulmonary disease (COPD). He has his own BiPAP machine for night-time ventilatory support. De Peri is receiving 1 to 2 litres of oxygen at home via nasal prongs. He is a compulsive chain smoker. No history of fever, chills, wheezing, chest pain, palpitations, distension, nausea, vomiting, or diarrhoea is available. Even with moderate exercise, he suffers respiratory difficulties.
