The patient has high blood pressure (BP 158/88) and is not on any routine medicines prior to this admission.
The RN returns and asks you for a summary. You explain what you have found, and the RN asks you to suggest 3 health care modalities that could support comprehensive care for your client.
You need to identify 3 multidisciplinary team members or support agencies that could support your patient. Explain to your assessor, who is playing the role of the RN, what each of these could contribute to the care plan and how you would establish a connection and collaborative relationship with each (e.g. what would you do to contact them, what would you say, how would you ask them to be involved, etc.).
