Your client is a 65-year-old Japanese man who is 180cm tall and weighs 50 kg. This English-speaking man and his wife were in Canada visiting their two adult children and suddenly suffered a stroke ten days ago which resulted in right-sided hemiplegia.
The physician's orders state: soft diet, activity as tolerated, and encourage mobility. The spouse is with the client from 0800-2000 daily. Breakfast trays will be arriving in 20 min.
As you enter the room, you see the client's dentures in a cup on the bedside table. A urinal is on the floor. You know from report that the client has been eating only ¼ of the meals and occasionally chokes on foods. The spouse states that the client is "looking much thinner and eats like a bird."
Identify both the effective and ineffective symptoms related to the nutritional assessment. Are there any assistive devices which may be beneficial to this client?
