Tyrone Jacobs is a 12 year old male patient who arrives in the emergency department with his mother and father gasping for air.   A nursing assessment reveals the following vital signs:

Temp:  37C

BP:  98/70

P:  115

R:  32

O2 sats:  95% on room air

Additional data obtained when he arrives in the ED includes:

Height:  4'5"

Weight:  40.5 kg

Medications:

Albuterol inhaler:  2 puffs as needed every 4 hours for shortness of breath

Singular:  5 mg by mouth daily

Another nurse completes the initial assessment, then hand's the client off to you.  During the handoff, the nurse tells you:

Tyrone presented to the ED gasping for air with an occasional non-productive cough.  His lungs are wheezy and he is using accessory muscles to breathe.    Tyrone feels short of breath.  He used his albuterol inhaler four times at home with no improvement.  He is alert and able to speak in full sentences but noticeably tachypneic when talking.  His skin is warm and dry.  Tyrone is getting over a recent upper respiratory infection and has clear rhinorrhea.  Denies sinus pain or pressure.  No hearing loss.  Ears free of cerumen.  No redness to the throat.
