Assignment task:
6-month old RJ is an infant whose mother is raising them non-binary until they decide for themselves. RJ was born at 39 5/7 weeks via SVD and growth and development have been appropriate. Mother lives in Oakland with the child in an apartment. Mother plans to return to work next month - just got a new job and is saddened to have to switch insurance and leave RJ's PCP (us). RJ has been well, exclusively breastfed, though mom is preparing to begin solid foods this month. Mother does believe in vaccinations in general. However, she has not decided whether/when she will completely follow the CDC guidelines for vaccine intervals and has decided to take an alternate approach. She is concerned that the baby's immune system might work too hard if they receive so many vaccines at once. To date, they have received DTAP #1 and Rotavirus #1 at 2 months, HIB #1 and PCV7 #1 at 3 months, DTAP #2 and Rotavirus #2 at 4 months, and HIB #2 and PCV13 #2 at 5 months.
How you might focus on the issues mom brings up. What you would recommend at today's visit? What approach for vaccination mom seems to be following. Today, if she agrees to receive some vaccines but not all, please be sure to explain which vaccines you would recommend today and why. What approach for vaccination does mom seem to be following? How would you focus on RJ's strengths? What else would you discuss at today's visit? What anticipatory guidance would you offer today? What follow-up would you recommend?
