Problem: How would you counsel this patient based on this (Day 1: RT is a 23 y/o F who visits today stating, "I need help. My sister convinced me to come in because I've been struggling with fentanyl." She states she's been using fentanyl over the past 1.5 years and uses daily with increased use during the holidays and on the weekends. She's tried to stop in past without success. She states she likes the feeling but knows she needs to stop using. RT says she lost her job and is alienated from many of her family members due to her use. Today she states her craving is an 8/10. She yawns frequently and seems agitated and anxious during her visit. She says she has been irritable and has not been able to sleep. She has complaints of aches and pains. PMH: RT states she has been using about three times a day for a year now. She was first exposed with friends 1.5 years ago at a party and additionally has occasional use of marijuana (first use 15 y/o), Ambien (first use 16 y/o), and psylocibin (unknown first use), but she prefers Fentanyl.
