Discussion Post: Research-Based or Evidence-Based Practice
The threat to patient safety and quality outcome are associated with healthcare-associated infections (HAI) incidents. In nursing evidence-based practices are utilized to provide quality patient care and reduce unexpected outcomes. Evidence base practice is the basses for interventions that reduce patient risks and hospitals acquire occurrences such as falls, ventilator acquire acquire pneumonia, catheter associated urinary tract infections etc. According to the CDC (Center for Disease Control) 75% of hospital acquired urinary tract infection are associated with urinary catheter. In my facility the nurses utilize the prevention bundle for the above-mentioned healthcare acquired infection or incident. The CAUTI bundle includes the indication for a foley catheter insertion, foley is no longer used for intake and output outside of the intensive care units the only indication is for urinary retention. Remove as soon as it is no longer necessary. Foley insertion is a two-nurse process to ensure sterile field is maintained, all Cather care is done with soap and water or CHG wipes once a shift or as needed. Best practice shows that there is a decrease in CAUTI when these practices are utilized.
One of the leading causes of injury related death in the elderly is fall. Falls are very costly to the patient and health care system. Implementation of hourly ensures patients' safety measures are met. To prevent patient falls, the nursing staff assess that patient's needs are, which includes the 5 P's potty, pain, position, personal items within reach and pumps. Evidence based practice shows that there is a reduction in the rate of falls when patients are rounded on frequently. These evidence-based practices are readily available to all nursing staff throughout the health center institution via the policy and procedure manual on the hospital website. In-services are another way staff are kept up to date on the latest research. The Up To Date is another educational tool whose usage is encouraged to bridge the gap between experience and current best practices.
Inconclusion evidence-based practice is the gold standard of care for best patient outcome and quality patient care outcome. Evidence base practice is utilized in various aspects of health such as reducing fall, CAUTI, VAP and pressure.
