Nursing Report: 6-week-old Caucasian female presented to the NICU lethargic, difficult to arouse, fontanels sunk in, tachycardic, poor eating, decreased weight from birth, and decreased muscle tone. Birth weight was 8 pounds 2 ounces. Current weight is 6 pounds 14 ounces. Social Services came in to see the child and family earlier in the day.
