My externship site is at a health information exchange company in Tennessee, and they receive data from all the local hospitals and clinics in the area. My company is tiny, and they outsource most of their employees through contracts to manage the day-to-day operations. The way they handle data from outside hospitals is that the hospitals send them the data to audit and make changes to the changes created and sent back to the hospitals. I would recommend that they make it mandatory for the hospitals to require specific fields when registering patients. One of the most significant issues I have encountered is not knowing whether a person is connected to a chart because the hospital did not collect enough data when registering. There are multiple people in the system with the same name, which makes it challenging to match the right person to the correct chart.
