In addition, please prepare a ONE PAGE fact sheet listing local services/providers of your area, including the name of services, eligibility requirements, points of access, phone number, and description of services provided, and anticipated outcomes.
Your presentation should begin with detailed description/characteristic of likely consumers of the services you are presenting on. Your presentation must include contact/point of admission personnel, phone number, websites, and any other information needed in order to make a possible referral.
