Problem
End Stage Renal Disease, (ESRD), is a very challenging diagnosis. Patients will likely at some point need diaylsis treatment. You will need to compare and contrast both "Hemodialysis and Peritoneal diaylsis" and include access ports. Explain how patients can be successful at each. And continue to lead a happy and productive life. Additionally, patients are required to continue to have routine follow up visits with their nephrologist. Include details on when to use the appropriate E&M codes for these visits.
