As my innovative model of care delivery, I chose Community-Based Solutions. As discussed in our book, It is well understood that social necessities, such as good food alternatives, transportation to appointments, housing, and poverty, impact an individual's health. Keeping patients healthy improves their quality of life while decreasing healthcare expenses. Effectively utilizing community resources and bringing people together helps to sustain these types of efforts. The Staten Island Performing Provider System (PPS) and Care Transition Choices (CTC) are two successful instances of community engagement. The first is an excellent illustration of how to improve a community's health and prevent admissions. As a health administrator, I think that educating patients and providing them with resources to help them make good lifestyle choices is one of the first steps toward creating a healthy community. Initiatives like the CTC are also critical since many people do not always recover well following major operations or other high-risk medical treatments. Having coaches to assist them in navigating these transitions is critical for the patient's health and minimizes readmission and expenditures (2019).
I would start by using the CDC's guidance for meaningful community participation in health and equity to design a program similar to those outlined above. The first stage would be to analyze previous initiatives and determine whether or not they were successful. Building trusted ties with the community and understanding the organization's ability to assist must be followed. It is not sustainable to expect more from people than they can offer. Community assessments, forums, focus groups, and interviews offer information on available resources and where initiatives should begin.
