Case study
A 46-year-old, 230lb woman with a family history of breast cancer. She is up to date on yearly mammograms. She has a history of HTN. She complains of hot flushing, night sweats, and genitourinary symptoms. She had felt well until 1 month ago and she presented to her gynecologist for her annual gyn examination and to discuss her symptoms. She has a history of ASCUS about 5 years ago on her pap, other than that, Pap smears have been normal. Home medications are Norvasc 10mg qd and HCTZ 25mg qd. Her BP today is 150/90. She has regular monthly menstrual cycles. Her LMP was 1 month ago.
The regimen recommended (which includes pharmacotherapeutics) is:
The diagnosis is that she does not have an infection, and that she has high blood pressure. that she takes Norvasc, and HCTZ, for the blood pressure.
First and foremost, if two forms of medicine are used to treat the same condition and those two drugs may (or will likely) lead to an elevation in deviations, these must be discouraged. This has to do with the fact that Norvasc and HCTZ are both medicines that deal with the issue of lowering blood pressure.
The other recommendations here, would be that:
• In one year, the pap should be repeated. It will be a regular/ routine examination to confirm or detect the absence of Ascus since she is either about to start her cycle or is in ovulation, which could influence the results.
• Norvasc should be discontinued, in which case its alternative would be Brevibloc. This is because the use of Norvasc alongside Hydrochlorothiazide will increase the occurrence of arrhythmia. Brevibloc, on the other hand, does not ideally cause an irregular heart rate
• Antibiotics such as clotrimazole are prescribed for her genitourinary discomfort and amoxicillin for her flushing and sweating (antibiotics for bacteria), as bacteria in her system could cause this.
