Assignment Task: Plan of care

The story

Sarah's family has a long history of involvement with Social Services, the result of which has seen all six of the children's names being added to the child protection register on three separate occasions in 1994, 1995 and 2000, under the category of neglect. This last period began in October 2007 when Sarah and her siblings were removed from the family home due to their mother's drug and alcohol abuse, sexual exploitation, parental absences from home and neglect. All children were placed into foster care in November 2008, which led to two of the children being adopted in July 2009. Sarah had a very strong emotional reaction to the adoption of her brother and sister, stating that the Children's Social Worker who had dealt with their case said that she would continue to see them when this was not what happened. This had a detrimental impact on Sarah where she believed she could not trust anything that she was told by professionals. Eventually Sarah was adopted along with one of her brothers. She was placed into a loving home of two professionals. She was attending school and despite her background was a high achiever. She was a member of the debate team and the speech meet club. She was also volunteering at a local animal shelter with hopes to be a veterinarian. However, the placement broke down when Sarah reacquainted herself with her mother. Sarah's mother was using drugs and associating with known drug dealers and gang members. Sarah was apprehended by authorities. Sarah was placed in a children's residential unit. Sadly, during her time here, Sarah tried to commit suicide by taking a drug overdose of Tramadol and Ativan. Sarah stated her reason for this was because she was because she was not allowed to be with her sister. Sarah was admitted to hospital and while she was there, she began to make significant improvements while working with her mental health team. Sarah was able to recognize the difficulties she experienced while growing up living with her mother. She wanted to better herself further by accessing more support networks. She wanted to return to school and live independently. However, Sarah still views her life as a daily struggle.
