Problem

Select a personal health behavior that you would like to change. Examples of health behaviors could include lifestyle changes to improve physical health (e.g. changing sleep habits, drinking more water, eating healthier, exercising more, etc.) or mental health (e.g. managing stress, engaging in healthy social relationships, etc.). Health behaviors can also include health-seeking behaviors (e.g. screening for a certain health condition, seeing a physician regularly, going to the dentist, etc.). Please also make sure that you choose a behavior that you are willing to discuss in various assignments during the semester.

Complete the following worksheet to specify how you would plan and evaluate an intervention to change the selected personal behavior within a rural population.
