Topic background: Many patients are prescribed a new medication during a period of hospitalisation. During this time, nurses are required to provide safe and quality care related to medication management. Task Description: Evaluate a case scenario and NIMC documentation and create a 5-minute video addressing concerns about digoxin administration in relation to the case. Case scenario It is 6 th September at 0900 hours. You are caring for Mr Joe Haldeman, aged 65 years. Mr Haldeman presented to the emergency department (ED) at 1400 hours yesterday with sudden onset of palpitations and severe dizziness. He has reported having diarrhoea for 3 days (his last bowel action was 1900 hrs 5th September). An ECG revealed atrial fibrillation (AF) at 158 bpm and he was placed on cardiac monitoring. He was commenced on a loading dose of digoxin (see NIMC). Mr Haldeman was given the first two doses of digoxin in the ED and arrived on your ward in a monitored bed at 0600 hours this morning. He remains in AF. Mr Haldeman has a past history of hypertension (12 years), type 2 diabetes (10 years), and irritable bowel syndrome (IBS; 5 years). He is prescribed Diltiazem CD 180mg and Teltartan HCT.
