Identify three situations in which familial traditions or personal beliefs were different than what the health care provider ordered or recommended.
For example: Did you ever choose not to take a medication that was recommended or prescribed?
Did you take a lower dose of medication or stop earlier than recommended?
Did you elect not to have a flu shot or other vaccination recommended by the medical community?
Have you made a decision to take care of a health concern on your own rather than seek medical attention?
Describe the situation, the actions you took, and the outcome of these actions. If you chose not to follow medical advice, how do you think this affected your relationship with the health care provider?
