Draw a decision table to represent the type of treatment

The following narrative summarizes an interview with an accounts clerk working for the EyeTunes Club.
Members of the Club whose order value exceeds $100 in a year and have a good payment record are given priority treatment. Where a membership has been over 5 years the member will get priority treatment if the order value exceeds $100 per year, in which case the club is prepared to overlook a poor payment record. If the order placed by a member of more than 2 years' standing does not exceed $100 during the year, but the member has a good payment record, then priority treatment is also given. In all other circumstances the order is given normal treatment.
Q1 A. Draw a decision table to represent the type of treatment to be given to a customer of the EyeTunes Music Club.
Q1B. Produce a structured English version based on the following dialogue obtained by the Systems Analyst from the client interview below:
"When orders arrive, they contain the customer's name and address, and, for each item in the order, the item-name and quantity required. I use the item names to look up prices from the price list, calculate and write the total for each item, and add them all up to get the order subtotal. Customers get a 10% discount if they are good payers i.e., their account status is shown as "A"-so I calculate this, subtract it from the order subtotal to get the order total, and add these figures to the order. Otherwise, the order total is the same as the ubtotal. Then I look up the customer's name to see if they have enough credit to cover the order-if not, the order is returned."
