Assignment task: Many times, when children lack the capacity or ability to represent themselves in a situation, a parent or guardian is able to sign on their behalf. Sometimes this is done through a consent form. The parent/guardian is giving consent for their child to act, participate, purchase, etc. Pretend you are running a business of your choosing in Ohio (gym, school, activity center, research facility, medical center, etc.) Draft a consent form for a parent/guardian that would allow a child to use your business.
