Robert's cardiologist agrees that the unit of red cells can be given over three hours and documents this change in the EMR. He also reminds the staff to be diligent in documenting Robert's input and output. He requests the CCU Registrar review Robert at 1700 hours and again at 1800 hours to ensure he is not showing any signs of fluid overload. Robert's nurse continues to monitor him closely for any signs of a transfusion reaction. Observations (TPR and BP) are taken 15 minutes after restarting the unit and then planned to be taken hourly, and again at completion of the red cell unit. What other information regarding the transfusion should be documented in addition to observations and the time the unit was commenced?
