While some recover spontaneously, up to 60% have chronic language impairments lasting beyond six months. Chronic aphasia significantly reduces quality of life, more so than stroke alone, Alzheimer's, or cancer. Severe aphasia in the acute phase, larger brain lesions, and older age at stroke onset are linked to worse chronic aphasia. However, current models considering these factors only explain about 50% of the variance in chronic aphasia severity, suggesting other unidentified neurobiological factors play a role.
