Problem:
A 54-year-old man presents for a routine physical exam .his last physical was about 10 years ago. History: he has no health care for 10 years, denies major illnesses or surgeries. His has a family history of diabetes and high cholesterol (mother, living, age 73), fatal myocardial infarction (father at age 64). He is uninsured with financial constraints and has work stress (12-hour days, 6 days per week. He has a 20 pound weight gain over past 5 years. On exam his BMI = 27.4 kg/m2, blood pressure: 145/85 mm Hg without evidence of end-organ.
