Case Study Pediatric Asthma
Aryn Patel; is 11 years old and was diagnosed with asthma when he was 8. He takes salmeterol 2 puffs PRN. For the last week he has had an upper respiratory infection. He came to the ER with uncontrolled coughing, wheezing, and was very short of breath.
For this critically ill child, Intravenous epinephrine was given to rapidly relaxes bronchial smooth muscles over one minute. IV prednisone was given and a combination of ipratropium bromide and albuterol was given and repeated once for persistent respiratory distress. Trevor was admitted to the pediatric wing for several days for observation and has done well. He is now being discharged and instructed to follow up with his doctor in 1 week.
Aryn is being sent home on :
• albuterol/ipratroium inhaler (Combivent) 2 inhalations 4 times daily for 1 week. (note: this is a combination drug and effects of both rx should be discussed)
• montelukast (chewable tablets): 5 milligrams (mg) once a day in the evening
• gualfenesin 100mg tab q4hr. PRN for cough
The provider has asked you to discuss these medications with Aryan's parents.
