Clinical Day 1 - This is a group activity. Due to the current Coronavirus concerns, you will have to collaborate with your clinical group members via phone, email, text, Google docs, Skype or any other technology. Please note that you will be doing a peer evaluation on each other for this activity so be sure to work together and share the workload.
This clinical homework will assist you in understanding the community as a client.

Watch this YouTube video to understand the background for community health, community assessment and the windshield survey. We will have you do a virtual windshield, but the video helps you understand the process. We have also posted a Windshield Survey guide on D2L too. Read through the guide to help you understand the assessment.

To do a virtual windshield survey you will have to access various websites and discuss findings with your group.
Here are a few sites to assist you in the data collection.

o Delaware Health and Human Services Guide for Aging and Disabilities.
This is not a comprehensive list of resources. You can also try other sites such as Zillow, city or town websites, etc.
You will gather data and extensively report on all the following items. You must provide the website hyperlinks for all references/citations for the data. Also, identify which group member, in parentheses, was responsible for each data item.
• Investigate the boundaries of your assigned zip code, not just the city/town. One member of the team should drive around the assigned zip code collecting observational data, i.e. condition of home, businesses and closures, industry presence, recreational facilities, schools, etc.
• Describe the history of one city or town in or near (if there is not one in the zip code) the zip code.
• Describe the zip code in terms of urban, suburban & rural environments.
• What type of housing is available (public, private, apartments, development, etc.)? Provide statistics on types of housing and ages of homes.
• Look up the demographics, population size and diversity and provide the data on the ethnicity, race, & gender.
• Provide data on the age of the population? List the breakdown across the lifespan.
• What is the income level for this zip code? Include single, female head of household vs. single, male head of household.
• What is the poverty level in percentage for this zip code and the State of Delaware? What is the federal poverty level in dollars. Consider how a family of four survives on this amount of income. Keep in mind that these dollars are pre-tax.
• What industry or small businesses are in the zip code?
• What is the air quality? What are the environmental risks? Obtain the rates of respiratory illnesses and cancers in this zip code.
• What are the crime rates and types of crime in the zip code?
• Are there any public parks, senior centers or other recreational facilities?
• How many healthcare facilities are in the zip code? Include hospitals, freestanding emergency rooms, nursing homes, walk-in clinics, surgery centers, etc.
• How may grocery stores/food markets, pharmacies, liquor stores & churches are there in the zip code?
• List all of the schools, public and private.
Your group will send one email with your data collection for the items above to your assigned faculty member. We do not require any specific format but you must answer each data point. If you are having trouble finding the data, try Google. Send this as an attachment in a Word document. In the email, state the group letter (A, B, C etc.), members of the group and give a summary of the time spent on this activity. This email is due Day #1 by 1500 for day students and 2100 for evening students.
Clinical Day 2
This is a group activity. Due to the current Coronavirus concerns, you will have to collaborate with your clinical group members via phone, email, text, Google docs, Skype or any other technology. Please note that you will be doing a self/peer evaluation on yourself and each other for this activity so be sure to work together and share the workload. This is the analysis of your data collection. After you have gathered the data for the questions above, your group will create and submit a voice-over PowerPoint and self-/peer evaluation to the appropriate homework submission folder in D2L. See Tips for PowerPoint Presentations below. The voice-over PowerPoint & Self/Peer Evaluations are due Day # 2 by 1400 for day students & 2100 for evening students.
The voice-over PowerPoint must include the following information:
1. Title: Project Title including the Zip Code (including a map of the zip code boundaries), Names of group members, and date.
2. A brief description of your assigned zip code. Describe the zip code in terms of urban, suburban & rural environments. Also, include a brief historical overview of the city/town within the zip code.
3. Provide the demographic information. The population size and diversity: provide data on the ethnicity, race, gender & age of the population. List the breakdown across the lifespan.
4. What is income level for this zip code? What type of employment opportunities are available? Consider industry or small businesses you researched.
5. Include the poverty data for this zip code? How does it compare to the state of Delaware?
6. What is the air quality? Discuss environmental risks and their health impact for the zip code. Report the rate of respiratory illnesses and cancers in this zip code.
7. Discuss the crime rates and types of crime found in the zip code?
8. What type of housing is available? Provide statistics on types of housing and ages of homes. Discuss why a community health nurse would be interested in this data.
9. What are the recreational facilities in the zip code? Why is this important? Think about health impact with limited recreational facilities in close proximity to where one lives.
10. How many healthcare facilities are in the zip code? Include hospitals, freestanding emergency rooms, nursing homes, walk-in clinics, surgery centers, etc. Do you think it is adequate? Why or why not?
11. How may grocery stores/food markets, pharmacies, liquor stores & churches are there in the zip code? Do you think it is adequate? Why or why not?
12. List all the schools, public and private. Think about the social determinant of education (or lack of education) and its impact on health.
13. What are the positive characteristics of this zip code? (on a separate slide)
14. What are the negative characteristics or needs for this zip code? (on a separate slide)
15. What are the health concerns for this zip code?
16. Select two top priority needs/issues/health concerns for your zip code and develop a plan to address these issues. Be creative and assume you have financial resources available. Imagine your group is conducting a community health and education fair for these issues. What resources are available, and what resources are lacking?
17. Discuss the group dynamics for this clinical activity. Discuss how you worked together.
You must also submit the self/peer evaluation (found on D2L Day 1 & 2 Clinical) on you and each of the members of your group to the D2L submission folder for this activity. Deadline is the same as the PowerPoint.
Format your homework according to the give formatting requirements:
• The answer must be using Times New Roman font (size 12), double spaced, typed, with one-inch margins on all sides.
• The response also includes a cover page containing the student's name, the title of the homework, the course title, and the date. The cover page is not included in the required page length.
• Also include a reference page. The references and Citations should follow APA format. The reference page is not included in the required page length.
