You notice a few things that make you question if the talk about having a just culture is just talk, especially when acute care is being provided to patients. Over the last year, the following set of circumstances has occurred:
SCENARIO #1: You overheard that one of the emergency nurses did not get written consent from a physician prior to giving a patient an emergent blood transfusion on a medical unit. There is currently no documentation in the patient's file with written consent from a physician regarding the transfusion. Obtaining written consent prior to transfusion is facilitypolicy. The nurse who gave the transfusion was reprimanded for not getting written consent.
SCENARIO #2: you leave the workplace at the end of the day, you notice that a biohazard bag is placed beside the trash can in the back parking lot. You have no idea how it got there, but you know there is a specific location to get rid of this kind of waste, and it is not by outdoor trashcans. The environmental services department has previously been reprimanded for inappropriately disposing of biohazard materials, but the issue keeps recurring
