Problem:

Barbara is a 28-year-old African American client who reports to the office with complaints of extreme fatigue, a loss of 15 lb in the past month, frequent low-grade fevers, and recurrent vaginal yeast infections. Her temperature in the office is 101°F, and her body mass index is 17. As you review Barbara's health history, you note that she identifies as cisgender and heterosexual and is sexually active. She has a history of gonorrhea infection and pelvic inflammatory disease. On examination, you note thrush in her throat and fine, dry rales on auscultation of her lungs. On performing a pelvic exam, you palpate uterine and adnexal tenderness and cervical motion tenderness. On questioning Barbara regarding her sexual history, she tells you that her last sexual contact was about 3 weeks ago. Based on Barbara's health history and symptoms, you suspect HIV.
