Problem: A patient with severe arteriosclerotic heart disease (ASHD) of native arteries, unstable angina, and severe chronic obstructive pulmonary disease (COPD) was admitted for coronary artery bypass graft (CABG) with cardiopulmonary bypass for two hours. An open 3-vessel CABG with the left internal mammary artery anastomosed to the left anterior descending coronary artery and separate greater saphenous vein grafts, harvested endoscopically from the left leg to the obtuse marginal branch and posterior descending coronary artery.
