Problem: The patient is a 23-year-old college student with a history of infectious mononucleosis at age 15. She presents to the clinic with a severe sore throat (11/10, "ice pick" characteristics), mild headache, and fever that started two days ago. She reports constant "swollen glands" in her neck, with intermittent headaches and fever, stating she feels "horrible." She had a temperature of 101.8°F, relieved by OTC acetaminophen. Currently, her temperature is 101.5°F. She reports her roommate had similar symptoms earlier this week. The physical exam reveals that the patient has pharyngeal erythema with tonsillar exudate, halitosis, swollen glands, and tender cervical lymphadenopathy. The nodes are 1.5 cm in size, rubbery, and mobile. She denies coughing, hoarseness, difficulty speaking, weight loss, and tobacco use but reports social alcohol use. The patient is up to date with her immunization but has not received her influenza vaccine. Her respiration is easy and nonlabored, with no adventitious breath sound and hepatosplenomegaly. She denies other symptoms.
