Problem:
Mary is a nurse working in a residential care facility. Mary's colleagues had noticed that the patients in Mary's care were looking uncared for over several shifts and that wet sheets were unchanged. Vital signs and some nursing charts were not completed. Mary left a patient's medical record unattended at the patient's bedside, and many times Mary failed to document it in the patient's medical records. Mary was also spending a lot of time at the nurses' station while her patients were left unattended. Mary's refused to help her team members when asked. Mary's colleagues have observed Mary's practices and reported their concerns to the manager. The management followed up on the concerns with Mary and, in doing so, found Mary was pretending to be a registered nurse.
