Assignment Task:

These are the instructions that the person responded to below

- Discuss the laws in your state related to the nurse practitioner's (NP) role and responsibility in creating and signing advanced directives (ADs). Indicate if the NP can independently complete AD documents or a physician is required to sign or cosign the documents.

- Consider clients you've encountered in practicum (or in your practice as a registered nurse). Identify at least ONE client who may have benefited from a referral to hospice or palliative care. Indicate why this client would have benefited from these services. Describe how as an NP you might approach the conversation with the client.

- Discuss at least TWO recommendations you have for improving palliative and hospice care access to vulnerable and underserved populations in your community.

Read this. This is what you are to respond to- The laws of California allow NPs to discuss clients' choices concerning treatment and explain the possibilities of end of-life care. According to Kandrack et al. (2021), they should document these decisions in ADs. A collaborative agreement or standing procedure with a collaborating or supervising physician is crucial. It guides NPs when developing and signing ADs unless they are utilizing full practice authority as stated under the California Nurse Practitioner Act starting January 1, 2023. While on my practicum, I came across a 72 years old client diagnosed with metastatic lung cancer and had been admitted severally due to extreme pain and respiratory failure.

As an NP, I would explain the situation gently and. I would comfort the client to affirm that they are heard and understood during such a conversation. According to Cai and Lalani (2022), barriers such as transportation issues, lack of local healthcare facilities, financial limitations, and community awareness hinder palliative care. Culturally appropriate education can challenge misconceptions and present knowledge on the utility of those services for the management of symptoms and quality of life. The use of mobile palliative care units or telehealth services would bring services directly to clients' homes. These recommendations serve to minimize barriers in delivery of services and guarantee that vulnerable and disadvantaged populations have access to timely and adequate treatment irrespective of the location and their income.
