Assignment Description: This case study assignment provides students with an opportunity to think critically and demonstrate their evolving understanding of relational inquiry in action, using a personal self-reflection approach. The reflective case study will be completed using a case scenario with guided reflection questions. This is an independent assignment which must be completed individually. The assignment will be submitted to the course professor through Blackboard using SafeAssign. Please refer to the Humber BScN Student Handbook (2024-2025) for further details regarding late or missed assignments.
Instructions:
1. The title page template provided must be completed and included with your submission
2. Ensure the case scenario and required course readings are used to frame your response to each question.
3. Responses to each reflective prompt must be typed, double-spaced, size-12 Times New Roman font, 1-inch margins, and must not exceed the allotted word count. Any content that exceeds the word count will not be graded.

Case Scenario:
You are a new-graduate, primary care nurse working in a small community health care center. Today you are seeing an 82-year-old widow who is accompanied by her adult son and daughter. The patient was previously an elementary school teacher and an active member of the community. She has a history of osteoarthritis, and breast cancer (currently in remission), and is currently experiencing a progression of Alzheimer's disease. She decided to move in with her daughter's family (husband and 1 school-aged child) 2.5 years ago, as she was unable to manage her home due to osteoarthritis
The patient's son and daughter accompanied her to the healthcare center today to discuss safety concerns. The daughter is concerned as the patient is becoming increasingly forgetful, wandering in the night, and experiencing a decline in functional skills such as feeding and toileting. Recently, the patient wandered outside while the family was sleeping and was confused about where she was when found. The daughter is sharing concerns with you today regarding her ability to continue to care for her mother and is inquiring about a potential application to a long-term care facility. The patient's son is aware of these concerns but does not want his mother to be admitted to a long-term care facility. He would prefer that she remain in her current living situation, with arrangements for additional in-home support
