Study the resources in your community related to the illness group chosen.
1. Identify and discuss a minimum of three community or national resources for your illness group preferably in your community.
2. Include in your response all the information the patient should know about the services.
a. Benefits
b. Specific services they offer.
c. Fee schedules, if and when appropriate.
d. Criteria for eligibility.
e. Application process.
f. Availability of transportation.
3. Discuss integration of these resources into a plan of care.
4. Discuss potential advantages and disadvantages for vulnerable populations in seeking these resources.
The response should include a reference list. Double-space, using Times New Roman 12 pnt font, one-inch margins, and APA style of writing and citations.
