Discuss hospital moving from paper-based record to ehr


Case Scenario- Your hospital is moving from a paper-based health record to a totally electronic health record. Your coders are currently on-site, but when the electronic record is in place, you want to be prepared to have the coders work remotely. You, the HIM director, must analyze and plan for the new workflow. However, before you can do that, you must be able to communicate the current workflow of the paper record so that you can compare it to a future workflow of an EHR. Task- Create a swimlane diagram that encompasses the path of the inpatient paper health record from patient discharge to its filing in the permanent file. To start, know that the paper records must be retrieved from the units. Once the records are retrieved, there are processes (assembly, analysis for example) that take place that are performed by staff who are not coders. The record is then coded and then there are other processes that take place (final chart check and completion, and filing for example) and are performed by staff who are not coders. Your swimlane diagram is going to show the progression of the record through these various processes and positions/departments. You may show more steps than I have outlined here or that are shown in the above links. If the links are not helpful to you or you are not familiar with the steps a paper record takes through an HIM department, you may need to perform an internet search or interview an HIM professional, colleague, or professor. On an additional or separate page, you are going to explain to your CFO the efficiencies gained by having the coders work remotely once the EHR is in place. Consider also the steps or resources that might be eliminated from the migration from paper to the EHR and how this will improve efficiencies elsewhere in the department or hospital.
