Problem: History and Physical 1545: History of hypothyroidism and cholecystectomy. Family states the client has seemed depressed and has been sleeping a lot for several days. Client is lethargic and somnolent. Skin cool and dry. Bradycardia at rate of 58/min per ECG. Respirations slow and shallow. Positive bowel sounds auscultated. Abdomen soft and nondistended. Peripheral edema noted 2 . Hair dry and brittle and loss of about a third of eyebrows noted.
