Discuss history of cardiac disease and hypertension

Problem:

The patient is a 33-year-old man with a history of cardiac disease and hypertension who suffered a full-thickness burn to his right foot three weeks ago. He was admitted for an elective wound debridement and for split-thickness skin graft. In the operating room, debridement was carried down to viable tissue. A 4×5-cm split-thickness skin graft was harvested from the upper aspect of the anterior right thigh. The graft was meshed and applied to the right foot wound. The graft site was secured, and the donor site was then dressed with Xeroform gauze. A posterior cast was applied to the right lower extremity, immobilizing the ankle Final diagnoses and procedure: (1) Full thickness burn to right foot, (2) cardiac disease, (3) hypertension, (4) split-thickness skin graft from right thigh to right foot. Looking for only 3 diagnoses code
