Students will:
1. Assess client families presenting for psychotherapy.
2. Develop genograms for client families presenting for psychotherapy.
To prepare:
1. Select a client family that you have observed or counseled at your practicum site.
2. Reflect on elements of writing a comprehensive client assessment and creating a genogram for the client you selected.
Task
Part I: Comprehensive Client Family Assessment
Create a comprehensive client assessment for your selected client family that addresses (without violating HIPAA regulations) the following:
o Demographic information
o Presenting problem
o History or present illness
o Past psychiatric history
o Medical history
o Substance use history
o Developmental history
o Family psychiatric history
o Psychosocial history
o History of abuse and/or trauma
o Review of systems
o Physical assessment
o Mental status exam
o Differential diagnosis
o Case formulation
o Treatment plan
Part II: Family Genogram
Develop a genogram for the client family you selected. The genogram should extend back at least three generations (parents, grandparents, and great grandparents).
Format your homework according to the give formatting requirements:
1. The answer must be double spaced, typed, using Times New Roman font (size 12), with one-inch margins on all sides.
2. The response also includes a cover page containing the title of the homework, the course title, the student's name, and the date. The cover page is not included in the required page length.
