Prepare 3 to 4 pages in which you complete the following:
1) Describe the community you have selected.
2) Explain the social change issue.
3) Differentiate between need and demand as they relate to the issue and community.
4) Outline/describe steps you would take to conduct a needs assessment.
5) State which stakeholders you would contact and why you would contact each.
6) Develop a survey related to your social change issue.
7) The survey must be at least 10 questions.
8) Provide a justification for each question on the survey.
9) Provide a rationale for the type/format of questions on the survey.
10) State how you would vary items on the survey based on the role of the stakeholders who would complete it (administration, leadership, staff, recipient of surveys).
Format your assignment according to the following formatting requirements:
i) The answer should be typed, using Times New Roman font (size 12), double spaced, with one-inch margins on all sides.
ii) The response also includes a cover page containing the title of the assignment, the student's name, the course title, and the date. The cover page is not included in the required page length.
