The U.S. Chemical Safety and Hazard Investigation Board (CSB) is responsible for determining the root cause of industrial accidents. Since its creation in 1998, the CBS has identified 83 incidents that were caused by management system failures. The accompanying table gives a breakdown of the root cause of these 83 incidents.
