Problem:
This 15 year old female presented to the hospital with an intrauterine pregnancy at 29 weeks gestation, premature labor, premature rupture of membranes times 4 days, chorioamnionitis, and lack of prenatal care. An ultrasound performed after admission revealed a vertex presentation. Because the patient had a temperature of 101.4 degrees, antibiotics were started. Following a prolonged second stage, a female infant with Apgars of 5 and 7 was delivered spontaneously over an intact perineum. The patient continued to receive antibiotics for three more days before being discharged home. She was determined to be anemic upon admission and began treatment with an iron supplement. icd code
