COMPREHENSIVE GERIATRIC ASSESSMENT (CGA) A Comprehensive Geriatric Assessment (CGA) is a diagnostic process that is also used for evaluation and management. A referral for a CGA can be completed by an older person's primary clinician (GP) or by a clinician caring for them in a hospital setting. The CGA identifies the medical, psychological and functional limitations and risks of an older person. The evaluation of physical, cognitive, social, financial, environmental and spiritual issues and the impact they can have on their health is also identified. 2 AGED CARE ASSESSMENT TOOL (ACAT) An Aged Care Assessment Tool (ACAT, or ACAS in Victoria) is an assessment that is used to identify how much help and what type an older person may require and connect them to appropriate services. It also determines if they are eligible for government subsidised aged care services. An ACAT is completed by a member of the ACAT team. This might be a nurse, social worker of other health care professional. 3,4 HOME SUPPORT ASSESSMENT Home Support Assessment is able to identify the needs of an older person to enable them to remain living at home safely.
