She initially discloses that she has come to your office because her husband is threatening to leave her if she doesn't "get help." She stated her "nerves are shot." When asked to explain, she stated that she likes everything to be "in order" and experiences significant anxiety when things are not in their place. She reported that she cleans her house from "top to bottom" every day. When her husband and children are home, she will follow them around the entire house, cleaning up the mess they make. In addition to her preoccupation with neatness, she also reported extreme anxiety related to their safety. More specifically, she stated she has to check to make sure all the windows are shut and locked, all the electrical outlets are unplugged, and the appliances are turned off, three times before leaving the house. She stated that this routine can take up to an hour before she feels comfortable to leave the house. In fact, she started getting ready 3 hours early prior to coming to this appointment. Because of these symptoms, she (along with her family) rarely leave the house as her husband gets annoyed with the amount of time it takes to leave. Janie reported wanting to stop these behaviors, but doesn't know how.
