In addition to using academic sources to help create a health action plan, consult with at least one professional in your community. The person you consult with could be a nurse or someone working within the health care field. You can do this via phone, in person, or through the Internet.
Think about what questions would be good to ask:
1. What are some current outreach strategies used with this population?
2. Are there local community resources that work with this population?
3. Are there other community resources that might help increase health awareness for this health issue?
4. What are the biggest challenges for health providers to address this community health issue?
Then, use this information to act as a nurse leader to develop a plan to deal with this issue.
This action plan should include the following details:
- Outreach.
- Describe strategies or techniques to promote health care prevention and disease maintenance within your identified population.
- Explain the cultural considerations that health care providers need to be aware of when working with this population and health care concern.
- Treatment or clinical setting.
- Describe the techniques and strategies that health care providers need to have in place when working with the identified population.
- What types of policies need to be implemented to enhance quality patient outcomes with this particular population and health concern?
- Communication.
- Describe collaborative communication techniques or policies that need be implemented when working with other health care teams to improve the identified health concern with the specified population.
- Describe types of communication techniques that could be implemented to reduce patient barriers to health care prevention or disease maintenance.
- Identify communication technologies that could be utilized to contribute to effective communication.
