After reviewing the agency's intervention/treatment plan, complete the intervention/treatment plan with a client with whom you are working.
Length: 1 to 2 pages.
I. Briefly describe the setting where the intervention/treatment plan was conducted.
II. Briefly discuss how you used the literature to guide your practice (i.e. actual use of the treatment/intervention plan).
III. Briefly summarize the experience of actually working with the client and any emerging themes (i.e., anything in the literature that captured your attention or was repeated throughout) that were present during the intervention.

Evaluation Program Transcript
Social workers link clients to programs and agencies to help them attain certain goals. The evaluation process is a tool that ensures this link is strong and allows social workers to demonstrate accountability. Much in the same way, your field instructor will evaluate your day-to-day practice skills when working with clients to determine whether your interaction with clients has improved or whether you can identify successful programs that meet the needs of a vast clientele. You will prepare evaluation activities as well as read evaluative literature so that you develop skills to effectively deliver services to your future clients.
