Jane Doe, a 24-year-old new mother, brings her infant boy, John, to the clinic for his twelve-month check-up. The nurse practitioner (NP) walks into the room and observes Jane playing with the boy. Jane appears happy and calm. The NP introduces herself and asks Jane the reason for the visit. Jane states they are here for the child's twelve-month check-up. The NP explains the steps of the physical exam and proceeds with the exam. Once the exam is completed, the NP explains to Jane she noticed in John's record he has not received his vaccinations, and there is no documentation as to why Jane declined them.

Jane immediately appears frustrated and states she has been through this with previous doctors and has concerns about vaccines causing serious problems. The NP calmly acknowledges her concerns and goes on to explain the vigorous testing for the safety and effectiveness of vaccines. Jane becomes defensive stating a few mothers in a support group she attends, were telling her vaccines cause developmental problems like autism. The NP calmly explains there have been several studies to discredit the claim. The NP continues to explain to Jane the importance of vaccines in preventing serious diseases and complications and the benefits outweigh the risks.

Jane insists she is being forced into this choice and feels disrespected by the medical professionals. The NP verbalizes understanding of Jane's feelings, informs her of the importance of feeling comfortable to make the right decision regarding John's health, and provides Jane with information and resources on vaccines. Jane appreciates the explanation and time but still needs to feel confident with her decision. The NP is supportive of Jane's need to feel confident and encourages Jane to take her time and call the clinic if she has any questions for her.

Describe the client's challenging behavior

Jane's behavior was argumentative. She developed frustration, distrust, defensiveness, and concerns over autonomy. Jane felt she was being forced into a decision she was not confident in making at that time. She has already expressed her concerns to other doctors and felt she was not respected or acknowledged for her concerns.

Examine the potential impact of the client's behavior on the client-provider relationship

Jane's behaviors can cause distrust in the client-provider relationship. Feelings that her concerns are not being heard. Effective communication and compromised care can also impact the client-provider relationship. If clients feel misunderstood or judged, they will be less likely to share their concerns and shut down. A strained client-provider relationship can compromise the quality of care.

Analyze techniques to enhance communication with the client and address the client's behavior

In this scenario, Jane was feeling frustrated and angry, which led to her being argumentative. A technique the NP can use to enhance communication with Jane is to patiently listen and empathize with her concerns (Tanoubi, et al., 2021). This allows for a trusting relationship between the client and the provider. Jane needs to be heard without judgment.

Another technique to use is identifying the reason for the frustration and showing empathy. This technique conveys to the client that the NP is attempting to understand and offer support (Tanoubi, et al., 2021). Jane became argumentative because she felt like her concerns about vaccines were being dismissed and her decision-making was being disrespected. Sitting down with Jane, discussing her concerns, and providing the education needed to support or discredit her concerns, is another technique to use (Tanoubi, et al., 2021).
