Describe persistent vaginal pain and vaginal spotting


Problem:

A 46-year-old woman is being seen in the office for persistent vaginal pain, vaginal spotting, and pain with urination for 4 months. The patient states that her male partner complains of penile pain with vaginal intercourse and the sensation of a sharp scraping. The patient's history is significant for a vaginal hysterectomy with mesh-augmented anterior and posterior repair 8 years previously. The patient has no medical problems and has been smoking one pack of cigarettes a day for 20 years. On examination, the external genitalia appear normal. On vaginal examination, there were several areas of approximately 0.3 to 0.4 cm in diameter of the posterior vagina of denuded epithelium showing some white net-like material exposed. There does not seem to be redness, swelling, or discharge. Question: What is the most likely diagnosis?
