Describe jane health status and physical condition

Assignment task:

Jane is in her 70s. Jane refuses to take her diabetic medication and she does not follow a diet as she loves to eat all the wrong foods such as pasta, white bread, french fries and cakes. Jane has advanced diabetes and broke her ankle in a fall 6 months ago (in part due to poor eyesight and loss of feeling in her legs). She now spends most of her time sitting on the couch knitting and watching TV. She puts all her snacks (potato chips) and food (cheeses to put on saltine biscuits) on her over chair tray to minimise walking to and from the kitchen. She drinks only a small amount of water daily. Her only regular exercise is using her walker to go to her letter box (about 3 metres from her door) to collect junk mail which she loves to read. Jane does not do the exercises the physiotherapist has recommended for her ankle and it has become stiff and not as flexible as it should be.

On reading her client notes you notice that her blood glucose level is elevated, her blood pressure is high and that she is complaining of feeling cold, lacks energy and just does not feel well.

2. Describe the impact of the disease on body system functioning.

3. List the indicators of physical and mood changes that Jane is experiencing and compare these to their expected normal functioning.

4. Describe the impacts of ageing and disability on Jane's ability to engage in their ADLs (activities of daily living)

5. How does the Jane's condition impact on their wellbeing?

6. How and when do you report Jane's changes in her condition and to whom do you report to?

7. Explain the procedures for working with a multidisciplinary health team to implement Jane's individualised plan, including escalating concerns with and changes in Jane's status.
