Appearance: David is well dressed for the weather and present during the session
Attitude: David is in denial yet cooperative and he was able to conversate
Motor Activity: David has trouble sitting still
Affect: David displayed appropriate affect during conversation.
Mood: David's overall mood was agitated because his family wants him to seek help at the VA hospital.

Processes: David thought process is clear and a bit confuse
Thought Content: David does not present with hallucinations or delusions. Denied suicidal thoughts, plan, or intent. Perception: David's perception is within normal limits. Orientation: David's orientation is a farmer at the ranch
Cognitive Function: David cognitive function of memory loss and confusion.
Abstraction: David admits he is having trouble answering the questions on his favorite game show.
Judgement: David's judgement is decreasing as the result he leaves the sometimes overfeed or not feed the animals at all. He at times leaves the doors to the animal opened Insight: David presented with impairment of insight with difficulty understanding
