Assignment task: Physical exam findings:
Appears stated age, in no apparent distress. Alert and oriented to person, place and year; unable to recall specific month or day. Long term memory is intact but short term memory testing reveals impairment as evidenced by inability to recall three objects after 5 minutes; difficulty identifying objects and performing calculations. Demonstrates difficulty generating typical speech patterns; CN II-XII intact with normal motor and sensory function throughout.
Test results are as follows:
CMP/CBC normal. Thyroid function testing normal. RPR/VDRL nonreactive. UA toxicology negative.
Imaging: CT non contrast cranium study shows diffuse cerebral atrophy without evidence of mass, infarct or bleed.
